Request a new or review appointment booking

If you are a new patient, please fill in the details below and submit a booking request. Your request will then be triaged by the specialist and you will receive an email with a link to choose a date and time for your appointment.

New patients will need a valid referral, noting that GP referrals should please be indefinite, and specialist referrals will only last 3 months from the date of first seen (for Medicare rebates).There after, if a patient needs to be reviewed, they will need a GP referral (for Medicare rebates).

Note that this is a specialist clinic for patients with cognitive problems, not general neurological problems. Referrals for general neurological problems should be made with other specialists through your general practitioner.
